Loan Modifications are for the specific loan only, the second trust would not be eligible to be combined with the first Loan Modification.  A refinance is the only option where both loans can be combined into one loan.  Be careful to ensure the cost of the refinance is offset by a reasonable break-even, I typically recommend 24 months (Cost divided by monthly mortgage payment savings).
